The common interpretation giving to this picture when it first showed up on the internets is "this is a gag where Natalie is pretending to be pregnant while reading "Profiles In Corruption" a book which has it's very own chapter on the Biden family.

I went to their concert in Austin, Texas while I was in college (1998-2000 timeframe).

They had a straight up commie activist take the stage for their set break. This was when Austin was still red, and she was booed off stage.
