Article 111 of the constitution says: When the leader, following illness or any other accident, is temporarily unable to carry out the duties of leadership, a Shura Council composed of the President of the Republic, the head of the judiciary, and one of the jurists of the Guardian Council performs responsibilities of the leader for the duration of the disability.

If Khamenei was sick, this council would be born. It has not.
