🔴 ECFMG on New Student Visa Appt Hold

We want to keep you informed of a breaking development that impacts J-1 physicians.

Current Situation:
The U.S. State Department has temporarily paused the scheduling of new student and exchange visitor visa appointments (including J-1 visas) at U.S. embassies and consulates worldwide as it develops expanded "social media screening and vetting." This directive was issued earlier today and is effective immediately.

What This Means:
Scheduling of new visa appointments is on hold temporarily!
The State directive indicated that existing visa appointments (already scheduled) will be honored; as such, incoming and current J-1 physicians with appointments should still be able to proceed as planned.
This hold on new appointments affects the broader student/exchange visa classifications (all F, M, and J holders), not only J-1 physicians

Current J-1s in the United States should NOT travel outside of the United States until the hold is lifted if they will need to renew their visa

Our Response: The State Department expects to issue updated guidance "in the coming days." In the meantime, we are monitoring this situation closely and will continue to do our best to support training programs as circumstances evolve. We will also alert J-1 physicians in the next few days of the situation, as well as our plans to help them navigate this shift.

We understand this adds another layer of complexity to an already challenging environment. Your professionalism and dedication to supporting J-1 physicians during these uncertain times is deeply appreciated.

📌NRMP Statement Regarding Recent Trump Administration Orders Related to International Citizens

June 6th 2025

The NRMP is issuing this statement in response to recent actions taken by the Trump Administration related foreign nationals.
On May 27, the U.S. Department of State instructed embassies and consular posts to pause the scheduling interviews/appointments for J, F, and M visa applicants. The pause extends to all those applying for J-1 visas, including physicians.
On June 4, the administration issued an order barring entry into the United States for foreign nationals from 12 countries – Chad, Congo-Brazzaville, Equatorial Guinea, Eritrea, Libya, Somalia, Sudan, Afghanistan, Myanmar, Haiti, Iran and Yemen – and partially restricting entry for citizens from Burundi, Cuba, Laos, Sierra Leone, Togo, Turkmenistan and Venezuela.
We recognize that international medical graduates (IMGs) who are seeking or have secured residency training in the United States apply for J-1 visas and may intend to travel to the United States from countries specified under the June 4th restrictions. We are working with ECFMG, a division of Intealth,  to understand the scope of impact for individuals anticipated to begin training on or around July 1 of  this year. Programs with matched applicants from a restricted country(s) may request a waiver of the Match commitment through the NRMP website.
We also recognize that IMG’s whose visa appointments are already scheduled and who have been cleared to proceed may be challenged to begin training on time. For those programs affected by the May 27th announcement, we ask you consider a delayed start or a one-year deferral of the match commitment in lieu of a waiver, if possible, to allow those IMGs facing prolonged delays the opportunity to finalize visa processing and move forward with their training. That said, we recognize the hardship this could impose, and programs requesting a waiver(s) may be granted if a matched applicant is unable to begin the program on time.
The NRMP will maintain regular communication with the ECFMG to stay apprised of any developments and monitor the situation. Questions about IMG visa application and processing should be directed to  the ECFMG. Questions about Match Policy should be directed to policy@nrmp.org.

The U.S. Department of State has lifted the pause on new visa appointments and has directed U.S. embassies and consulates to prioritize J-1 physicians for visa interview scheduling.

This means that embassies can now resume booking visa appointments and have been asked to give priority to physicians applying for the J-1 visa for educational exchange.

🩺 OET Medicine (Required for All Applicants)
• Must take OET Medicine on or after January 1, 2024
• Applies to first-time and revalidation applicants



📚 Pathway-Specific Eligibility

1️⃣ Pathway 1 – Licensed Physician; License must have been valid on or after Jan 1, 2021

2️⃣▶️5️⃣ Pathways 2–5 – School-Based Options Graduation must be on or after Jan 1, 2023

6️⃣ Pathway 6 – Mini-CEX Evaluation Must involve real, outpatient, non-relative patients – no emergency, inpatient, or family/friends allowed




What You Can Do Now
• Schedule your OET Medicine exam (date must be after Jan 1, 2024)
• Prepare required documentation (licenses, evaluations, school letters, etc.)
• Monitor ECFMG.org and MyIntealth for updates



Pro Tips
• Don’t delay OET booking – scores are required before rank order list deadline
• If using Pathway 6, carefully follow new restrictions for clinical encounters
• Have your materials ready to apply as soon as the portal opens

📚 Pathway & Graduation
• Iran:
• Students enter medical school directly after high school.
• Internship lasts ~18 months and includes Internal Medicine, Surgery, OBGYN, Pediatrics, and rotations in subspecialties.
• After graduation, physicians are licensed general practitioners who can work immediately without residency (except for certain specialties).
• USA:
• Students complete a bachelor’s degree, then medical school.
• After graduation, they must complete residency to practice independently (no general practice license after medical school).
• No single long rotating internship like Iran; specialization begins immediately.

🧪 Focus & Training Style
• Iran: Broad practical training during internship; emphasis on clinical service, inpatient management, and hands-on procedural work early.
• USA: Strong early focus on standardized exams (USMLE), research, and academic evaluation; specialization-focused training begins sooner.
