Once the TI tells the psychiatrist of the symptoms being experienced and the psychiatrist understands and is in agreement with it, the TI should not wait for the psychiatrist to put it in their medical record.

I made a copy of the last month's conclusion on Havana Syndrome by the government and a copy of my UN Paper on Cybertorture and a copy of my US State Dept. Havana Syndrome questionnaire and told the psychiatrist that I need to place it all in my medical records to support my diagnosis to prove I’m not crazy.

My Havana Syndrome diagnosis was also based on my $1 online self-background checks and a $75 Private Investigator background check on my social security number that showed my straw identity was being used as an alias; that my real name had been changed to the alias as a frame up for blackmail. The alias name began showing up in my mail as advertisements. I’m happy to answer any questions you have."
