It's documented in a 285 page Senate Report released by Sen. Chuck Grassley & Ron Johnson that everyone else just seemed to miss.

FBI Asst. General Counsel Kevin Clinesmith prepared the warrants used to spy on @RealGenFlynn and the Trump Transition Team using The Logan Act as the basis. Judge James Boasberg signed one of the warrants. Boasberg is the same judge that later let Clinesmith walk for falsifying evidence used in obtaining the Carter Page FISAs.
