In UK Column yesterday they were discussing palliative care “currency” & asked for help on just what this is. If anyone would like to send this post to UK column please do so, as it is a first toe dip into looking into this, upon which others can build. Reference to, & some commentary is taken from “The Adult Specialist Palliative Care (SPC) Currency Guidance Document” linked below.

Apparently, health Commissioners are already familiar with the concept of “currencies” & their use but most specialist palliative care providers have little or no prior experience of currency models and their use in commissioning and paying for health services.

The currency project for palliative care started as a result of the Palliative Care Funding Review which published its final report in 2011, with 3 key aims being “to create a fair & transparent funding system”, “to deliver better outcomes for patients”, & “to provide better value for the NHS”

These aims were to be achieved by developing:

• A funding system based on need
• A funding system which incentivised good outcomes for patients irrespective of both time & setting
• The commissioning of integrated care packages to stimulate community services.

It was widely recognised that there was a general lack of good data about the palliative care sector. Therefore, a data mining exercise was authorised to help “achieve the above aims”. Data was collected from the sector to develop “currencies, with a target to collect information about 7,000 specialist palliative spells of care. The data collection pilot ran from 2012 & was completed in May 2014. It therefore seems like a lot of modelling has gone into this program, from data collected from patients. There is no mention as to whether patients gave their consent to this.

Analysis on the data led to the design of a “development currency”. This eventually led to the publication of “A New Approach to Palliative Care Funding” in March 2015 which set out the proposed development currencies, (which I see as nothing more than a cost benefit analysis), for adult, children & young people’s specialist palliative care.

Currency consists of two elements, case mix, & the resources needed to deliver that case mix. “For many providers collecting information about the resources associated with particular activities can be more challenging & will depend to some extent on whether your accounting practices allow you to collect costs at a patient level. The data collection templates discussed in the section ennoscriptd, “How to Use the Currency - a Practical Guide” is supported by further detail in Annex 2 & will help you to formulate an approach appropriate to your organisations circumstances”.

The above is very troubling to me. It is an exercise in “average cost per phase for acute hospital in-patients, or hospice in-patients; & an average daily cost for community settings”. Therefore, I see it as an exercise in determining the costs of looking after extremely sick & vulnerable people and whether "this is worth it". This is dangerous, as it reduces a person to a number & the cost to society at large, for their care. I will go out on a limb here & say this kind of attitude is what is driving the push towards legalising euthanasia. It seems clear that there is a pervasive belief at some level, that when someone can no longer work or contribute to a system hell bent on punitive taxation measures, they are regarded as a drain on society.

I personally feel this currency for care is nothing other than a social score given to you by the government. (interesting in light of social credit system – maybe this is the precursor to it, based on health & contribution to society). The higher your score the more useful to society and/or compliant you are, the lower the score, the less useful to society and/or compliant you are.

Page 9 of the attached document evidences this very well with relative costs for the care of those of “unstable health” being of course higher, that those that are of “stable health”.
