A 12-year-old girl who died after an incident of self-harm was failed on multiple levels, an inquest jury has found.
Mia Lucas, who died in January 2024, was found unresponsive at an NHS children's psychiatric unit after developing a rare neurological disorder that had been left undiagnosed.
The jury at Sheffield Coroner's Court heard the girl was found at the Becton Centre, part of Sheffield Children's NHS Foundation Trust.
She had been placed there after being sectioned while suffering an "acute psychotic episode" during an assessment at the Queen's Medical Centre (QMC) in Nottingham.
On Thursday, the jury found that the failure to undertake a lumbar puncture at QMC before her transfer to the Becton Centre "possibly contributed to Mia's death".

A man who threw a six-year-old boy from the 10th floor of the Tate Modern has been found guilty of attacking two nurses at Broadmoor hospital.
Jonty Bravery kicked a female nurse in the thigh and "clawed" another in the face, leaving her with blood dripping down her cheek.
The 24-year-old was given a life sentence for throwing the French boy from a balcony of the London art gallery in 2019.
The boy survived the 100ft (30m) fall, but was left with life-changing injuries, including multiple broken bones and a bleed on the brain.
Bravery is now at Broadmoor, a high-security psychiatric hospital in Berkshire.
He chose not to attend his trial at Westminster Magistrates' Court on Thursday, where he was found guilty of assaulting nurses Kate Mastalerz and Linda McKinlay in September last year.

Electric vehicle owners have reacted with dismay to another tax being slapped on their eco-friendly cars after the Chancellor announced plans to impose mileage-based electric Vehicle Excise Duty (eVED) charges from April 2028.
It will be charged at a rate of 3p per mile, and 1.5p per mile for hybrid car owners.
New car owners will be offered the option of estimating their first year's mileage in order to set their initial rate.

An NHS screening programme for prostate cancer could come one step closer if it's backed today by a key committee that advises the government.
One in seven men with prostate cancer actually have a normal PSA level.
And even those with a high level may have a cancer that is so slow growing that it's just not a threat.
That's why the National Screening Committee has warned in the past that PSA screening could lead men to have surgery or other treatment that they don't actually need. Treatment can result in incontinence or impotence.

A mugger who murdered an 87-year-old man as he walked home with his shopping has been jailed for a minimum of 23 years.

Attempted rape charges against two Romanian teenagers, that sparked widespread racially-motivated riots across Northern Ireland, have been withdrawn.
The boys, aged 14 and 15, were accused of attempting to rape a schoolgirl in Ballymena, County Antrim on 7 June.

Renfrewshire Council confirmed that the child attended Paisley Grammar School.

Sir Chris Hoy has voiced his disappointment after mass screening for prostate cancer was not recommended for use on the NHS.
The National Screening Committee (NSC), comprised of doctors and economists, told the government that screening is "likely to cause more harm than good".
