while the child is in this paralyzed state of horror - the handler extracts the adrenochrome from the child by injecting a long syringe through the eye socket, where the handler must then pierce the child's pineal gland to extract the adrenalized blood & fluid that oozes out, and it all must be done in one movement; in-suck-out.

the lifeless but alive child is then typically used immediately for occult ritual sacrifice, bloodletting and cannibalism - as the blood inside of the child remains alive and adrenalized. the entire pineal gland is then cut out of the child's head, which resembles a hulled walnut that they offer to the host, then all share and ingest during these ceremonies.
