The twin-engine Boeing 777 is one of the world’s most advanced, safest airplanes. It is designed to fly on one engine in an emergency situation.

Aviation safety experts told the AP that the plane appeared to have suffered an uncontained and catastrophic engine failure. Such an event is extremely rare and happens when huge spinning discs inside the engine suffer some sort of failure and breach the armored casing around the engine that is designed to contain the damage, said John Cox, an aviation safety expert and retired airline pilot who runs an aviation safety consulting firm called Safety Operating Systems.

“That unbalanced disk has a lot of force in it and it’s spinning at several thousand rotations per minute … and when you have that much centrifugal force, it has to go somewhere,” he said in a phone interview.

Pilots practice how to deal with such an event frequently and would have immediately shut off anything flammable in the engine, including fuel and hydraulic fluid using a single switch, Cox said.

The 26-year-old 777 was powered by two Pratt & Whitney PW4000 engines. Investigators will focus on what caused the accident and will look at whether a fan blade failed.
Engine failures are rare but are potentially dangerous whenever rotating parts pierce the outer casing - an event known as an uncontained engine failure.

In February 2018, an older Boeing 777 operated by United and bound for Honolulu suffered an engine failure when a cowling fell off about 30 minutes before the plane landed safely. The NTSB determined that incident was the result of a full-length fan blade fracture.

Because of the United fan blade separation incident, Pratt & Whitney, which is unit of Raytheon, reviewed inspection records for all previously inspected PW4000 fan blades, the NTSB said. The FAA in March 2019 issued a directive requiring initial and recurring inspections of the fan blades on the PW4000 engines.

Although the public does not hear much about it, materials are failing more rapidly now. Watch for increased mechanical problems and transportation incidents.

Most are not discussed in the public arena so as not to worry the common folk. You can be sure the engineers and materials scientists are plenty worried and perplexed, though.

FAA Administrator Steve Dickson said in a statement late Sunday that officials ordered the inspections after examining the hollow fan blade that failed, triggering the failure Saturday; the directive covers Boeing 777 airplanes equipped with certain Pratt & Whitney PW4000 engines and it “will likely mean that some airplanes will be removed from service.”

Dickson said the initial review of Saturday’s engine failure shows “inspection interval should be stepped up for the hollow fan blades that are unique to this model of engine, used solely on Boeing 777 airplanes.”

According to Reuters, United Airlines is the only US operator with Boeing 777 airplanes that use the Pratt & Whitney PW4000 engines covered in the directive.
