👌Upper cell is called large granules lymphocyte ( LGL )

👌Down cell is called orthrochromatic erythroblast ( late normoblast)

🖋To different between large granules lymphocyte and monocyte that contain granules by the monocyte granules is fine granules but the large granules lymphocyte has coarse azurophilic granules.

🖋The large granules lymphocyte is at orginal NK cell

This cell is called plasmocytoid lymphocyte
This cell is position between the small lymphocyte and plasma cell .

Presence this cell in blood film with reactive lymphocyte and other lymphocytes kind associated with increase Total WBC count and with absolute lymphocytosis is indicated for viral infection ... More common in infectious mononucleosis

👌في حالة عد DC احنا نعتبر هذا لمفوسايت ...

ولكن في حالة دراسة فلم الدم لازم نفترق لان التشخيص يعتمد عليها لأنه في بعض الحالات هذه الخلية ممكن تسبب سوء التشخيص في حالة myeloma

اذا لم نفرق النوع هذا عن الانواع الاخري بنعتبر هذه الخلية عادي وهي في الاصل خلية تسمى plasma cell

وجودها بإعداد كبيره تطن انها إصابة فيروسية لسوء التشخيص ولكن هي في الأصل تعتبر neoplstic process

من المعروف لدينا عندما نعمل الفحص التفريقي DC COUNT
نعد لحدود مائة خلية ونشوف نسبة كل خلية كام ولكن في حالة تواجد هذه الخلية هناك طريقة أخرى تنضاف بعد العد التفريقي ..

👌بعد العد التفريقي اتضح لنا وجود ارتفاع في الخلايا الليمفاوية في هذه الحالة نقوم بعمل عد ولكن هنا العد يختلف

نقوم بعد كل الخلايا البيضاء بشكل عام في نقطة عد واحدة ونستخدم نقطة عد ثانية فقط لخلية reactive lymphocyte and plasmocytoid lymphocyte

ونشوف كم نسبة الخلايا reactive lymphocyte and plasmocytoid lymphocyte

من نسبة الخلايا البيضاء كاكل

If the ratio of plasmocytoid lymphocyte and reactive lymphocyte more than 10% this is more suggest for infectious mononucleosis

If the ratio of the reactive lymphocyte and plasmocytoid lymphocyte less than 10% is other viral infection

👌Right cell is call band Eosinophil

👌The nucleus of neutrophil has clumping chromatin pattern so that we see the color of nucleus is more deeply from other cell

👌The other cell has cytoplasm with dense brown granules

👉🖋important Information about MCH and MCHC and MCV .

👇👇👇👇
🌐In some anemia decrease or increase in cell size ( MCV) is associated proportional with
Decrease or increase in amount of Hb within cell ( MCH) resulting is normochromic cells.... Normal (MCH)

🌐In other anemia however the decrease in amount of Hb within cell ( MCH) is substantially more than decrease in cell size and cell appearance hypochromic..(decrease MCHC) so that it is important to understand this concept because microcytic cells with MCH less than 28 pg are not necessarily hypochromic and macrocyte with MCHC more than 34 are usually normochromic .

🌐Only RBC that is hyperchoromic with MCHC more than 36 g/dl is spherocyte.

👌So that MCHC is more specifically in describe the the hypochromic and normochromic RBC
If the results of MCHC is less than normal this indicated for hypochromic cells vice versa

س.لمادايفضل الهيبارين ك مانع تجلط في فحص asmotic fragilityولا يستخدم الاديتا؟؟👇


👌يفضل الــ. Heparine علي موانع التجلط الاخري لعدم احتوائه علي املاح حيث EDTA,trisodium citrate تحتوي علي املاح تؤثر علي الحامضيةPH .... بالتالي يمكن ان تعطي نتائج خاطئة ...
