요즘 퐁퐁수술 메지온이 핫한데 간단히 정리

1. 몸의 순환은 두 개인데 고압의 체순환, 저압의 폐순환이 있고 좌심실 우심실이 각각 담당

2. 단심실 환자는 하나가 없으니 체순환된 피를 곧바로 폐순환으로 연결 - 폰탄 수술

3. 근데 체순환이 고압이다 보니 저압의 폐순환이랑 정맥이 감당 못하는 문제 발생

4. 비아그라는 혈관을 확장시켜 영차 하게하는 약물인데 sildenafil이 동일한 기전 약물. 혈관 확장시켜 압력을 떨어뜨려 3번 문제 해결

5. 현재 허가 위한 재임상 진행 중. 내년 중 완료 기대

글로벌 퐁퐁 심장내과 의사들이 임상중인데 만나보면 톤이 상당히 확신있음

NICE는 진행성 HER2 low 치료제로 엔허투 사용을 거부했는데, 이건 비용 때문으로 해석

DESTINY-Breast 04 시험에서 화학요법 대비 엔허투가 PFS 4.8개월 더 연장하고, 전체 생존 기간도 6.4개월 연장하는 것으로 나타난 바 있음.

NICE(영국 국립보건임상연구원) 가이드라인은 영국에서 사용되나, 캐나다, 독일, 이탈리아 등 유럽 국가에서도 상환됨.

이번 딜의 시사점은???

1. Series A 단계의 초기 회사도 딜이 충분히 가능하다

초기 단계라 딜을 못한다는 건 이제 핑계임. 초기 단계여도 회사의 향후 개발 및 운영 전략에 따라 조기 라이선스를 해야하는 경우가 있으며, 요즘 같이 바이오 장이 힘든 시기에는 어떻게 보면 '선택'보다는 '필수'가됐음.

2. Opdivo 원개발사와의 면역항암제 개발 딜

Ono Pharmaceuticals는 Opdivo 원개발사이고, 넥스아이는 PD-1/L1 타깃 면역관문억제제의 내성을 극복할 수 있는 신약을 개발하는 컨셉이기 때문에 시너지가 좋을 것으로 판단.
고기도 먹어본 사람이 먹는다고, 이 경험은 절대 무시할 수 없음.

3. 국내 바이오텍의 수준이 점점 높아진다

요즘 비상장 바이오 회사들이 글로벌 제약사들과 딜을 이뤄냈다는 소식이 자주 들림. 빈도가 점점 높아지고 있으며, 계약 상대방의 티어, 계약 규모 등도 점점 수준이 높아짐.
아직까지도 상장사 바이오텍 중 상당수는 유의미한 임상 또는 기술이전 실적이 없는 상황이며, 앞으로도 있을 것 같은 회사는 많지 않음.
오히려 왠만한 상장사들 보다 비상장사들의 기술 및 사이언스 수준이 훨씬 높다고 생각되며, 이런 회사들이 많아짐에 따라 바이오 투자 환경은 점점 나아질 거라고 생각.

사실상 CAR-T 불모지인 국내 항암제 시장에서, 국내에서 개발된 첫 CAR-T 제품의 상업화가 예고되고 있습니다. 큐로셀(Curocell)이 면역관문분자 발현을 낮춘 CD19 CAR-T ‘안발셀(anbal-cel, CRC01)’의 림프종 임상2상 탑라인(top-line) 결과가 도출됐습니다.

큐로셀은 이 결과를 바탕으로 오는 9월 식약처에 해당 적응증에 대해 안발셀의 허가신청서를 제출할 계획입니다. 안발셀은 T세포의 억제성 면역관문분자인 PD-1과 TIGIT 발현을 낮춘 후보물질이며, 국내 바이오텍이 개발한 CAR-T로 최초로 임상에 들어간 후보물질이네요.
