📘 Lesson:
After several years of treatment with levodopa, many Parkinson’s patients develop motor fluctuations, such as “wearing off”, where the medication effect declines before the next dose. This is part of the “on-off phenomenon”. Management includes strategies like:
Adding a dopamine agonist (e.g., pramipexole, ropinirole)
Using MAO-B inhibitors (e.g., selegiline, rasagiline)
COMT inhibitors (e.g., entacapone) to prolong levodopa’s half-life
Shortening dosing intervals or switching to extended-release formulations may also help. The goal is to smooth out dopamine stimulation and reduce fluctuations.
