[키움 허혜민] 흥미로운 AZ의 타그리소 연구 결과 발표(Feat. 유한양행 경쟁약물)

타그리소 FLAURA 임상시험과 실제 환자 결과가 달랐음.

비소세포폐암 1차 치료제로 타그리소 투여 받은 실제 환자(’20.1~’21.10월 311명)

이 중 44%(137/311)은 ECOG 수치가 낮아 FLAURA 부적격 환자였고, 생존기간 중앙값(mOS)도 FLAURA 보고된 것보다 1년 더 짧았음. (16개월 vs 34개월)

다만, FLAURA 적합 환자는 임상 결과와 유사한 전체 생존을 보임

전체 27.4개월 대비 부적격 환자의 mOS는 18개월 짧았음.

흠.. AZ의 타그리소 가치 평가를 한다면, 1차 치료 환자의 절반은 OS가 더 짧다는 부분이 감안되어야하겠네요.

사실 근데 ECOG가 문제라면 이건 사실 타그리소만의 문제는 아니고, Ami+laz 뿐만 아니라 chemo 등 모든 치료법에 공통적으로 해당될 것임

아무래도 부작용이 적어 투약 부담이 적은 약물일 수록 유리해지는건 사실이겠다만, 마리포사 G3는 음…

원래 그런진 잘 모르겠으나, 경험적으로 임상 테스트할 때는 ECOG 0또는 1만 받던거로 기억

지금부터는 키트루다 SC 가치는 충분히 반영되었고 향후 나올(것 같은) ADC 및 시밀러 SC 가치 땡겨오기 멀티플 주기 싸움인듯

독일 머크(Merck KGaA)가 C4 테라퓨틱스(C4 Therapeutics, C4T)와 표적단백질분해(TPD) 약물 개발을 위해 총 7억5600만달러의 딜을 체결했습니다. 특정 암유발 단백질들을 대상으로 2개의 TPD 약물을 개발하는 내용의 파트너십입니다.

머크는 이번 딜에 앞서 지난 2022년 영국 앰피스타(Amphista), 오스트리아 프록시젠(Proxygen)과도 TPD 개발딜을 체결한 바 있으며, TPD 분야에 투자를 지속해오고 있습니다.

요즘 퐁퐁수술 메지온이 핫한데 간단히 정리

1. 몸의 순환은 두 개인데 고압의 체순환, 저압의 폐순환이 있고 좌심실 우심실이 각각 담당

2. 단심실 환자는 하나가 없으니 체순환된 피를 곧바로 폐순환으로 연결 - 폰탄 수술

3. 근데 체순환이 고압이다 보니 저압의 폐순환이랑 정맥이 감당 못하는 문제 발생

4. 비아그라는 혈관을 확장시켜 영차 하게하는 약물인데 sildenafil이 동일한 기전 약물. 혈관 확장시켜 압력을 떨어뜨려 3번 문제 해결

5. 현재 허가 위한 재임상 진행 중. 내년 중 완료 기대

글로벌 퐁퐁 심장내과 의사들이 임상중인데 만나보면 톤이 상당히 확신있음
